My understanding from contacting them is that you can expect a high percentage of downloads, but it's going to cost a minimum of $0.25 per download. You will also need about a $5,000-$10,000 initial investment. Finally, it really only works if you are promoting a free app, as you can imagine that the people looking at the ads are hence using a free app themselves and are unlikely to pay for an app through an ad.
The good news is that it isn't based on number of impressions. So if they see your iAd impression and then click on it, that is what counts against your total. You will of course get people that won't download after doing that, or tapped it by accident and don't care. But of the 20,000 taps you get for 5K, you can expect a high number of downloads.
For that price for a free app ... you might be better off going to FAAD. Unless you know that your free app can rake in enough to recoup the 5K from only 18,000 new users.
