I'm the CTO for Apptentive, which makes it easy to collect in app feedback and engage directly with your customers. We have 2 components (which take less than 15 minutes to integrate) and which may be of interest to you:
First, our ratings component provides the familiar "are you enjoying using this app" dialog with a twist: those users that indicate that they love the app are taken to Apple to provide a great rating, while those are unsatisfied in some way are asked to provide feedback.
Our feedback component (which the ratings component ties into), provides a simple, in-app dialog for a user to directly provide feedback to a developer, including attaching a screenshot.
All feedback submitted includes context and device details to ease debugging and inform your response, and you can also add additional custom app specific data to the feedback. Finally, you receive email notices of all feedback and can either directly reply from your email client or you can use our rich web interface for deeper details and analytics.
