iPhone 4 Personal Hotspot issues

Hi guys I was wondering if you could help me.

I have had personal hotspot ever since I've had my iPhone 4 and I use it regularly, today I go to use it and it isn't there (this has happened before so I go settings > network > turn on personal hotspot) HOWEVER it doesn't let me. I get the following error (see attached photo)




Any help anybody on here could offer me would be greatly appreciated.

I know this is an old track, but for other readers, your Mobile Phone provider has to support this. There is usually an additional charge from them to enable this.

Yes and it's included in my plan. I've used it for about 6 months and all of a sudden it's stopped working.
