1. Edit all this information in itunes connect under the app name. There is a bunch of required fields, but most of it's self explanatory. Not sure whether or not the website/email displayed to the customer is mandatory, make sure you make a separate e-mail from your personal one if so!
2. For updates, you'll add a new version in itc from manage your applications. It's the same application, but a different version and build number. Most users will be notified by updates automatically on their device.
3. Put this under review notes in itunes connect, or a link on how to create a login/pw.
4. Nope. My first app was approved and it was leaking 3 MB at a time in certain spots. Leaks are all too common and they don't have the time to track minor things like that.
