How Long To Make Simple Excel/Form Program

I am trying to figure out the projected cost of creating the following program:

• It will have a form with input areas. It is simply made to fill in the form and send to someone.
• The form will have 10 input areas asking for 10 different things.
• After they send the form to an email, it will say sent and then go back to the original screen

That is essentially the program. I am just trying to get a rought estimate as to how long a program like this will take. I am assuming 5-20 hours. What do you all think?
