Send a link for a page within an app

This may be easy, difficult or impossible. Either way, I haven't seen or read anything on the subject.

Basically, could you grab the "URL" on any mobile app - I assume each page of an app has a unique identifier or one could be created - and let's say, post it to Twitter so it looks like a shortened URL. So when someone clicks on the link within Twitter's mobile app they either a) automatically go to the page in the mobile app that was created or b) go to the app store if the user doesn't have the app downloaded.

What says you? Thank you in advance. Any help is appreciated!

I am no expert so I could get proven wrong but I would guess no since you can't guarantee the person clicking the link has the app

Thank you. But couldn't we tell if they have the app? If they did we could redirect them and if not the app store can be launched and the app downloaded.
