Simple iphone app development: form to email

Hi,

We want to have a simple app for sending fax from iphone, using our email-to-fax facilities.
We need a simple interface look like a web form for users to input data/text/photo attachment, then send all info to a specific email address which deliver the fax later. Some filtering rules needed to limit the fax number (the ban list), and a counter to limit the number of pages a user can send out.

We have graphic designer, all graphics will be provided.
