How can i register paid account on appstore??

I am from Pakistan....Currently Working as Freelance iphone developer....
I want to know how can i register a paid account on app store without credit card.

I have a debit card... Is there any possible way to create a paid appstore account so i can upload/work on my own ideas

need Step by step detail If possible.....

You can use debit cards too.
