Not sure what platform they are hosted on, but, I use bluehost.com and never had a problem. They support MySQL 5. Very reliable, all in all, a great company in my experience.

For small things I've used jaguarpc.com, good service and solid host.
For scaling up I'm using amazon AWS and that's a bit expensive but it gives you all the power you need.
I actually privately have sold web-hosting for almost 10 years now. If anyone would like anything i'd be happy to help, we have servers in the UK, France & USA.
Just a warning - keep away from ServerPronto. They'll lure you in with a bargain price then suddenly jack it up and they won't let you cancel without going through some ludicrous process of sending them photocopies of your credit card and driving license for "security" purposes.

I recently made some updates to my app that required server connection I made a .net service that is running on https://appharbor.com/ cost for hosting including ms sql 0$ works very well so far.
Of cause it is a cloud thing so wont work for everything.
000webhost is a good free host, they allow a lot of things sql php and the such. bad thing though is that they have safe mode enabled, so some scripts might not work. such as ones that write to other files. good if you want a basic site that just advertises your app. also, its ad free
hostgator is hands down the best paid host. idk if they still do it but they used to have it to where you could get awesome hosting for .01$ a month with some coupon code. nonetheless you can still get top of the line hosting for 9.99/month
I tried 000webhost for a little while, and their downtime was more than I could handle. Maybe it was on purpose, but when I made a big change to my website, the FTP transfers would keep failing and trying to start over. It was probably OK for a small site for a small app, but be ready to move when things get serious.
I've had accounts at Bluehost.com and Dreamhost.com for a couple years now and don't have complaints about either of them--however I don't know that they offer Windows hosting.
For cheap .NET / IIS hosting I have an account through AccuWebHosting.com, it's only a few bucks a month for a shared server.

Depends on your budget. I would suggest you take look at Rackspace Cloud Servers. Do not settle for cheap hosting services because... well, you get what you pay for.
I use BlueHost for all my web sites, and I have been very happy with them (for over five years now).
We use Mediatemple for our main site. It's not been great, especially recently. They've had some severe downtime that's accelerated our plans to switch to a different host. I'm sorry but having our primary portal down for 6 hours without even an email is unacceptable.
For Handoff and our store, we use Heroku. Heroku rocks for Rubyists, but is a little on the spendy side. They've got a plugin for everything, though, and require very little maintenance. Plus they handled getting Fireballed like pros, so we're pretty happy with them.
