Already was rejected with similar circumstance, so have re-submitted a modified version addressing their concerns.

Has anyone had one of these 'extension' emails, and then been approved?

Yeah my first app got 2 of those emails and took a month and a half to finally get on the store. Never got rejected though.
So I had 2 apps take only 2 days last week to get accepted. I submitted one Monday, however, and still In Review Friday... Really need it up this weekend.
